can failure from a few things:
1)Improper balance. You can't perfectly balance a 60 degree v6, but I do recommend "over" balancing them.
2)Harmonics, thats why dampeners are cool boys and girls
3)Clearances, thats why you check your shit before you put it together.
4)Oil pressure/viscosity. That is why I run Redline 10w30, slippery stuff and protects similar to a 50 weight oil. Thats my I have crushed oil squirters in all of our blocks and shimmed relief valves. Great oil pressures without having to take up "slop" in gears.
5)Bent cranks. Make a crank, mag it twice

. I had a 1.5" crack around #6, .5" around #5 and 1/4" crack around rod journal #1

. I mag'd the crank for shits and giggles after 8k of 9000rpm fury at 25+psi.
Thats really about it. Mostly its harmonics/rpm related, sometimes its lack of oil.

Jon Wieman (LethalVR4) put a brand new pump on a brand new factory block. The oil pump seized
during the first start up. The oil pump housing cracked and the crank snout was damaged. Everything was checked, but opening the oil pump. Mitsubishi wouldn't warranty the oil pump and pay for the crank and labor unless they opened the oil pump and found it was their fault. Jon asked if a neutral third party could open it and Mitsu declined. Jon asked to be present with an expert and his lawyers when Mitsu opened it and they declined. Jon considered pursuing things legally, but gave up because it wasn't worth the hassle. He just had another big f#ck you from Mitsubishi to put on the list of times they've screwed him.....that list also has his transfer case they wouldn't recall.
I personally installed a new oil pump as part of a 120k on one of Bob Forrest's DOHC non-turbos. I checked the bearings and they were fine, so I didn't replace them. The pump worked fine for a few hundred miles, but then it fragged itself. The case was cracked like the gears had displaced themselves. All clearances and anything else related were checked on that motor and were fine. The pump just fragged.
The only thing not checked on these motors was the engine balance. I could see bad balance causing the frag on Forrest's car, but the bearings were fine after 120k. I see no way that balance could frag Jon's pump. All he did was start it.
I did install about 6 other pumps around this time and did not have a problem with any of them. However a friend with a 99 Eclipse GSX fragged his race motor in a similar manner. His parts guy admitted a few pumps had come back recently. They gave him a new pump, but would not pay for his race motor. AMS would not warranty it either, even though they built the motor and put the pump on themselves. How's that for a kick in the nuts?
Anyway, that's my two cents on the oil pump failures.
